Sera Prognostics, Inc. (“Sera Prognostics,” “we” or “us”) is committed to protecting the privacy of all visitors to the Sera Prognostics website. Sera Prognostics has established this Privacy Statement to inform you of the specific practices and guidelines that help ensure the security and confidentiality of your personal information. By using or accessing in any way the websites we control and operate, including SeraPrognostics.com and PreTRM.com (our “websites”) or by transmitting information to us by email or other electronic means, you agree to the terms of this Privacy Statement. Sera Prognostics may revise this Privacy Statement from time to time. All updates to this statement will be posted on this Web page. Aggregate Data Collection
Sera Prognostics tracks visits to our websites and uses visitor logs to compile anonymous aggregate statistics. This aggregate information is collected site-wide and includes anonymous website statistics. In addition, when you browse our websites, our system automatically collects information such as your web request, Internet Protocol (IP) address, browser type, browser language, domain names, referring and exit pages and URLs, platform type, pages viewed and the order of these page views, the amount of time spent on particular pages, the date and time of your request, and one or more cookies that may uniquely identify your browser. This information is used to analyze trends, administer the websites, improve the design of our websites, and otherwise enhance the services we provide.
Certain pages of the websites and/or HTML email correspondence may use session cookies, persistent cookies, or web beacons to anonymously track unique visitors, save website preferences, and to allow us to recognize visits from the same computer and browser. Types of Personal Information Collected
Sera Prognostics may collect, store, and use personally identifiable information (such as name, email address, postal code and email preferences) when it is voluntarily submitted to us, such as when you register for updates or contact us.
The Sera Prognostics websites are directed toward adults. If you are under the age of 13, you must obtain the authorization of a responsible adult (parent, legal custodian, or teacher) before using or accessing our websites. We will not knowingly collect or use any personal information from any children under the age of 13. If we become aware that we have collected any personal information from children under 13, we will promptly remove such information from our databases.

We may use your personal information to contact you, improve this site, provide you with information that you have requested, or provide you with additional information, which Sera Prognostics believes may be of interest to you. Information that you provide to Sera Prognostics through these websites is encrypted using industry standard Secure Sockets Layer (SSL) technology. Your information is processed and stored on controlled servers with restricted access. Unfortunately, we cannot ensure or warrant the security of any information you transmit to our websites, and you do so at your own risk. As a consequence, Sera Prognostics disclaims any warranties or representations relating to maintenance or nondisclosure of private information.
Information Access, Updates and Choice
You may choose to provide information to Sera Prognostics by completing the registration form, sending us an email, or otherwise contacting us. In the registration form, you may have an opportunity to elect to receive certain communications from us. Sera Prognostics email correspondence will include instructions on how to update certain personal information and how to unsubscribe from our emails and postal mail correspondence. Please follow the instructions in the emails to notify Sera Prognostics of changes to your name, email address, and preference information. Sera Prognostics will take reasonable steps, such as confirmation emails, to verify your identity before granting access to your personal information. If you choose to unsubscribe from our email and/or postal mail services, you will no longer receive this correspondence. Ask us to correct your medical record
- You can ask us to correct health information about you that you think is incorrect or incomplete. Ask us how to do this.

Request confidential communications
- You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address.
- We will say “yes” to all reasonable requests.
Ask us to limit what we use or share
- You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care.
- If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.
Get a list of those with whom we’ve shared information
- You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why.
- We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly
Choose someone to act for you
- If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.

We will not retaliate against you for filing a complaint.
For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions.
You have both the right and choice to tell us to share information with your family, close friends, or others involved in your care.
We never share your information for marketing purposes unless you give us written permission.
In the case of fundraising, we may contact you for fundraising efforts, but you can tell us not to contact you again.
Note that we never market or sell personal information.
Our Uses and Disclosures
How do we typically use or share your health information?
We typically use or share your health information in the following ways.
We can use your health information and share it with other professionals who are treating you.
Example: A doctor monitoring your pregnancy speaks to us about your overall health condition.
Run our organization
We can use and share your health information to run our business, improve your care, and contact you when necessary.
Example: We use health information about you to process the order for your test.
Bill for your services
We can use and share your health information to bill and get payment from health plans or other entities
Example: We give information about you to your health insurance plan so it will pay for your test.
How else can we use or share your health information?
We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as public health and research. Help with public health and safety issues
We can share health information about you for certain situations such as:
- Preventing disease
- Helping with product recalls
- Reporting suspected abuse, neglect, or domestic violence

We can use or share your information for health research.

We will share information about you if state or federal laws require it, including with the Department of Health and Human Services if it wants to see that we’re complying with federal privacy law.
